What will you do
- Develop individualized plans for prenatal, postnatal, menopause, and chronic-condition care.
- Conduct one-on-one counseling sessions—assess dietary habits, set goals, and monitor progress.
- Collaborate with care teams (OB/GYN, endocrinology, nursing) to integrate nutrition into treatment pathways.
- Lead educational workshops and support groups on topics like breastfeeding nutrition and bone health.
- Monitor outcomes using KPI dashboards; adjust plans based on lab results and patient feedback.
- Maintain up-to-date records and document interventions in the patient portal.
